Union County’s Human Services Agency is working in collaboration with Novant Healthcare and Atrium Healthcare to complete the 2019 Community Health Assessment (CHA).

To help ensure this important information is obtained, we request that you take about 10 minutes to personally complete the CHA survey by clicking the appropriate link below.

The CHA provides a foundation for improving and promoting the health of Union County residents.  Primary data for the CHA is collected from surveys and focus groups.  Survey responses are anonymous and will be compiled with all responses received for each age group to help determine local health and safety practices and better understand the physical, mental and environmental health needs of our teens, adults and senior adults.  Survey data will help our community health and wellness partners determine the County’s health service needs, the level of service needed, and locations in the County where they are most needed, with the ultimate goal of ensuring availability of resources within the County to adequately address these needs.
